Healthcare card
In 2024, Care & Youth Caribbean Netherlands (ZJCN) introduced a physical healthcare card on Bonaire, St. Eustatius and Saba. The healthcare card is the replacement of the paper insurance certificate. Previously, you had to collect an insurance certificate from the ZJCN office before travelling abroad. With the introduction of the healthcare card, this is no longer necessary.
What is mentioned on your healthcare card?
- Your first name, surname, ID number, date of birth and place of residence (Bonaire, St. Eustatius or Saba).
- The contact details of ZJCN for rapid communication in the event of medical emergencies abroad.
When do you use the healthcare card?
- In the event of unexpected medical emergencies abroad, where treatment cannot wait until your return to your island. In that case, please contact ZJCN as soon as possible.
- Locally at GPs, hospitals, pharmacies and laboratories on Bonaire, St. Eustatius and Saba.
Collect your healthcare card during office hours at the office of ZJCN
To make it easier to collect your healthcare card, you can now visit the office of ZJCN at Kaya Grandi 91 during our regular opening hours:
- Monday, Wednesday and Friday: from 08:00 until 12:00 o’clock
- Tuesday and Thursday: from 13:00 until 16:00 o’clock

It is also possible to schedule an appointment in advance. Send an email to zorgpas@zorgverzekeringskantoor.nl and indicate your full name, sedula number and telephone number. We contact you as soon as possible to schedule an appointment.
What should you bring to your appointment?
- Your sedula (or passport for minors).
- A completed client form.
Can you not attend in person? (Collection through authorisation)
If you are unable to attend in person, someone else can collect the healthcare card on your behalf. This requires the following:
- A signed authorisation form
- The original sedula of the insured person (or passport in case of minors).
- The identity document of the authorised representative.
